Who Can Qualify and Benefit
The program serves individuals diagnosed with all forms of dementia, such as Alzheimer's disease, along with their primary caregivers. Whether someone is in the early stages of cognitive decline or facing advanced dementia, GUIDE offers tailored support that meets them where they are.
Unpaid caregivers — often family members who suddenly find themselves managing complex medical decisions while processing their own grief — receive dedicated assistance. GUIDE acknowledges that supporting the caregiver is just as important as caring for the patient.
To qualify, you must have a dementia diagnosis, be enrolled in Medicare Part A and B (excluding Medicare Advantage) and live at home or an independent or assisted-living facility.
